This registration session will allow you to complete contact information, sign up for participation and submit payment or agree to a payment plan. To complete registration through our secure site, please have your Visa, MasterCard, Discover, or checking account information available. Following the completion of this registration, you will see a charge on your account. You will also have the ability to pay offline by cash or check.
Player's equipment, travel, food and drink expenses are not included and are the responsibility of the parent/player for each event. All players are required to have a USLacrosse membership valid through July 1, 2018, which is NOT included in the Ballistic registration fee. Please see below for instructions on becoming a USLacrosse member. A Code of Conduct form and Consent to Treat form (see below for forms) are required to be on file with Ballistic Sports Group before any player is allowed on the field.

Boys will be placed on a team based on their age as of August 31, 2018. For example, if your boy is 12 years old as of August 31, 2018, he is eligible for the 12U team, and you should indicate that division when asked later in this registration. CLICK HERE to view an age bracket chart to help you determine your players division.
Girls will be placed on a team based on their grade level for the upcoming school year. For example, if your girl will be in or entering high school for the 2018-2019 year, she will be eligible for the HS team, and you should indicate that division when asked later in this registration.
Your player may play up to the next age level if our coaching staff determines he or she has the skills and mindset to do so, but at no time may a player play down a level. Any change in division level will take place after practices begin.
